If you received an introductory packet from us, fill out the new member form and return it using the included postage-paid envelope.
Already enrolled in the AccordantCare Program?
Activate your MyChart account for 24/7 support:
- Request an activation code by calling us at 1-866-360-5658, or request an activation code online
- Once you receive your activation code (or if you already have one), activate your MyChart account
What is MyChart?
MyChart is included in the AccordantCare program and allows you to:
- Get in touch with your dedicated care team, anytime, anywhere
- View and manage your care plan
- Find trusted information specific to your condition, all in one place
